A Care Manager can be a trained Social Worker,  Occupational Therapist or Nurse. You may be referred to a Care Manager if your dementia begins to make it difficult to do the day-to-day things you would ordinarily be able to do.

 

The Care Manager works with you to assess your needs and identify your personal outcomes. If you are eligible, they may arrange support such as Self-directed Support (SDS).

 

A Care Manager may also work with you if there are concerns about risks to you or others.

 

As your dementia progresses, you may need to arrange support to help you make decisions, for example Guardianship. The Care Manager works with you to identify what is required to support you. They may also involve a Mental Health Officer.
