Mandy, Enablement and Support Coordinator
An Enablement and Support Coordinator is sometimes known as an ESC. An Enablement and Support Coordinator works with the Care Manager and a range of professionals to assess needs and provide holistic support to people with dementia.
An Enablement and Support Coordinator isn’t a Social Worker. But they are qualified in a health and social care related discipline. They assess and coordinate support packages so you can be as independent and well as possible. They also identify if you may be eligible for longer term support.
